I, the parent/guardian of the above named player candidate on a team, hereby give my approval to participate in any and all organizational activities. I assume all risk and hazards incidental to such participation, do hereby waive, release, absolve, indemnify and agree to hold harmless the Junior Eagles Basketball Club, participants and persons associated with said club during club activities. For any claim arising out of an injury to my child, whether the result of negligence or for any other cause, except to the extent covered by accident and liability insurance, which is supplemental and subordinate to my own insurance. I agree to comply with the code of conduct set forth by the league. I will furnish a certified birth certificate of the above named candidate to club officials when required or requested. Further, I do hereby agree that the player will play with any team to which I am assigned by the club. The club reserves the right to limit membership to anyone determined by the Board of Directors to be detrimental to the well-being of the league.

By registering my child with Junior Eagles Basketball Club, I agree that I, and all individuals attending Junior Eagles activities on behalf of my player—including parents, grandparents, relatives, family friends, and guests—will represent the club with respect, sportsmanship, and appropriate behavior.  Treat all players, coaches, officials, volunteers, and spectators with respect. Do not argue with, harass, threaten, or disrespect coaches or officials. Do not make negative, inappropriate, or disrespectful comments about other players, children, coaches, families, or teams. Allow coaches to coach and officials to officiate without interference. Concerns or disagreements should be handled privately through club leadership, not through public confrontation or social media. Model positive sportsmanship and behavior for our athletes.   ZERO TOLERANCE  Junior Eagles Basketball Club has zero tolerance for abusive, threatening, intimidating, hostile, or disrespectful behavior toward officials, coaches, players, volunteers, or spectators.  Violations may result in disciplinary action, including removal from an event, suspension from attending games or club activities, or other consequences determined by the Junior Eagles Board.  By submitting this registration, I acknowledge that this Code of Conduct applies to me and all individuals associated with or attending activities on behalf of my player. I understand that violations by myself or my guests may result in consequences for attendance at Junior Eagles events.  I acknowledge that I have read, understand, and agree to this Code of Conduct.

Payment Method

Installment Option (1st Installment of $100 due 9/2 & 2nd Installment of $125 due by first practice on 10/4). First Installment is a non-refundable uniform fee and will be used to secure player's uniform.
